Job Description:

Summary: The Medical Record Clerk is responsible for the maintenance and control of all patient medical and dental records; producing files for use on the request of authorized personnel; protects patient and employee confidentiality at all times.

Essential Duties/Responsibilities:

  1. Chart Analysis - Multiple steps to process charts is put through prior to returning it to the permanent file area.
  2. Report Filing - Scan reports into electronic file.
  3. Record Release - Release record after following appropriate confidentiality guidelines.
  4. Archiving - Removal of inactive charts from permanent file area, relocation to offsite location.
  5. Receptionist - Answer phones, operate FAX machine and assist Providers with records as needed.
  6. Audits - Department is regularly called upon to participate in audits to assure compliance and quality of the medical record and patient care.
  7. Scan all documents into eCw, dating and naming them. Double check to be sure it was scanned into the correct patient’s account and scanned under the correct tab.
  8. Check electronic fax / Jellybeans.
  9. Function at highest level according to credentials and competency
  10. Positively impact patient experience by demonstrating values of Transforming Care including, but not limited to, courteous and helpful behavior and a commitment to accuracy.
  11. Share accountability for overall patient health outcomes, working in coordination with Care Teams.
  12. Operate to instill confidence in our care and in our facilities to patients, fellow employees, and other stakeholders.
  13. Other duties as assigned.

Physical Charts Only

  1. Chart Location - A chart when removed from the file area has an out guide put in its place designating chart destination.
  2. Pulling of Chart - Pulling chart for patient visit, prescription, audits, studies, etc.

Experience and Skills:

Qualifications:

Skills: Attention to detail required. Strong organizational skills required. Computer skills required. Bilingual (English/Spanish, English/Arabic) preferred.

Education: High school diploma or GED required.

Experience: One year experience working in healthcare preferred.
